As a member of Cadbury at Home, you will be assigned a personal care coordinator. Your coordinator is available to provide you individual attention, 24 hours a day, seven days a week. Whenever you need help - whether it is a home-delivered meal or the assistance of a home health aide - all it takes is a phone call to your personal care coordinator and they will handle the rest. Your personal care coordinator, working with you to maintain your independence, is at the heart of Cadbury Continuing Care at Home. As one member discovered, with other long-term care options, she had to find and arrange for the care she needed herself. This can be very stressful when you're already not feeling well.

 

What can you expect from your personal care coordinator?

  • A personal relationship with you that is vital in providing responsive, personalized long-term care services.
  • Periodic home visits in the comfort and privacy of your surroundings to address your concerns and care needs.
  • The coordination of an individualized program of services for you. Your personal care coordinator will schedule any service you will need thus alleviating any worry and confusion for you and your family.
  • A professional relationship with your primary care physician to discuss your treatment needs and develop an appropriate plan of care.
  • A resource for any of your long-term care needs and questions.

Any combination of these services will be provided to you on either a temporary or a permanent basis to help you stay as independent as possible, for as long as possible.
